Assess risk of current and future risk of T2 Diabetes and cardiovascular disease associated health conditions

Criteria for Full NHS Health Check as follows-

-Age 40-74

-Not already had a NHS Health Check within the last 5 years.

-Not already diagnosed with High blood pressure (and taking relevant medications), high cholesterol (and taking statins) or Diabetes.

-Living, working or GP located in B&NES.

NHS Health check includes the following screening-

BMI= Height and weight.

Blood Pressure

Cholesterol

HBA1C (blood glucose level)

Lifestyle screening also discussed, including ways to reduce risk and onward referral to our friendly and knowledgeable practitioners for ongoing 1-1 support provided if required. 

eligibility does apply but for those who do not meet the criteria can be offered the mini version
